To receive counseling:
Single-call intervention offered to all enrollees regardless of insurance status. Multi-Call (C-5):13 yrs+, Uninsured, Medicare, and Medicaid. Behavioral Health: 7 calls (C-7). Pregnant/Postpartum:10 calls (C-10). Consent required for services and call backs.
To receive medication:
Registered and uninsured, underinsured, Medicare, Medicaid receive free NRT ranging from 2-week starter kit to 12-weeks of combo NRT products. Pregnant/breastfeeding or under 18 yrs not eligible for medication.
Additional Info:
Individual Services Program allows participants to choose a menu of individual items that best meet their unique needs, such as asking for just a 2-week starter kit of NRT patch, gum, or lozenge without having to enroll in the full counseling program.
